Step 1: Call the office at 808-536-2196 or select 'Appointment Request' to schedule an appointment.
Step 2: Click the 'New Patient Registration Form' link below to complete 2 forms (Patient Registration, Health History)
Step 3: Confirm your appointment via text or email.
Please let us know 48 hours in advance if you need to change your appointment time.
Step 4: We get to Meet you!
- Arrive 10 minutes prior to your appointment time
- The doctor will see you first for a Comprehensive Exam and any necessary x-rays
- You will come back for your professional dental cleaning (possible same day appointments are available)
