New Patients

Your first visit to our dental office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. 

To understand what to expect for your first visit to our practice, please read through this page.  Please save time at your first visit completing the patient forms in advance of your appointment.

Step 1: Call the office at 808-536-2196 or select 'Appointment Request' to schedule an appointment.

Step 2: Click the 'New Patient Registration Form' link below to complete 2 forms  (Patient Registration, Health History)

Step 3: Confirm your appointment via text or email.

Please let us know 48 hours in advance if you need to change your appointment time.

Step 4: We get to Meet you!

  • Arrive 10 minutes prior to your appointment time
  • The doctor will see you first for a Comprehensive Exam and any necessary x-rays
  • You will come back for your professional dental cleaning (possible same day appointments are available)
